- Patients receive additional care and services in the home setting to achieve optimal outcomes.

- Patients will have access to services that may include Nursing, Telehealth, PT, OT, Speech Therapy, Disease Case Management programs, Respiratory Therapy, Medical Equipment, Enterostomal Nursing and Behavioral Health
- Homecare Triage nurse available to take after hours phone calls
The Ed U-Turn process involves initial agreement and coordination between:
- Emergency Department Attending Physician
- Patient's PCP
- Home Health Clinician
Patients with the following
low risk diagnoses
may be considered for the Global Ed U-Turn process:
- Wound Care
- Congestive Heart Failure (CHF)
- Chronic Obstructive Pulmonary Disease (COPD)/Asthma
- Urinary Tract Infections (UTIs)
- Falls and chronic ambulation problems
- Altered mental status with no acute documented alteration in the ED and no acute findings in the ED workup
- DNR patients requiring palliative care
- Hypoglycemia (non-recurrent) in Diabetics
