Bariatrics Program

The Bariatric Program is designed for bariatric weight loss surgery patients. A registered nurse focuses on lifestyle changes; teaching diet, exercise, wound care, and pain management according to the bariatric surgeon's instructions. Outcomes demonstrate a decline in the readmission rate for patients receiving home health services through the Bariatric Program.

Program Design:

• Our patients will receive an admission visit within 48 hours in receipt of final referral or as ordered by the physician.
• On admission, the Home Health RN will assess the need for additional services including:
• Mental Health Nursing
• Registered Dietitian
• Medical Social Work
• Rehab Services
• Gastric Bypass Program maximizes patient outcomes by continuing the physician’s protocol in the home.

Goals of the Program

• Promote self-management of post-op gastric bypass patients
• Reduce hospitalizations
• Increase patient compliance with nutritional plan
• Assist patient and family in managing lifestyle changes

Enhanced Care in the Home

• Educate patient on drains, wound care, intake & output, nutrition and potential post-op problems.
• Assess and make recommendations for pain control, current medication regimen and nutritional risks.
• Provide ongoing counseling support and initiation of appropriate community resources.
