Fast Track Rehab

Fast Track Rehab promotes self-management of the lower-extermity orthopedic patient and the patient who has had a stroke. A "Fast Track" patient receives a visit from a physical therapist within 48 hours from receiving the final referral - usually within 24 hours.

The same physical therapist will provide the patient with continuing care at home after the initial assessment. This means that you won't have to waste any time during home care visits explaining what the "last guy" did.

The Fast Track program improves patient outcomes by using the most advanced technological treatments according to the physician's protocol and national standards.

Who is Fast Track for?

Fast Track is for patients who have had certain kinds of leg surgeries. These include:

• Total Knee Replacement
• Total Hip Replacement
• ORIF (Open Reduction Internal Fixation) to repair a broken bone.
• Patients who have had a stroke

Enhanced Care in the Home

• Each patient receives the initial physical therapy visit, as directed by the physician, within 48 hours from receiving the final referral - usually within 24 hours.
• Patients receive continuing care from the same physical therapist after the initial assessment.
• Fast Track maximizes patient outcomes by continuing the physician's protocol in the home. The protocols may include range of motion, gait training, strengthening exercises, and activities of daily living, as guided by the physical or occupational therapist.
• Dedicated weekend and holiday coverage is available.
• Fall Prevention Program includes a fall risk assessment on all adult patients.
• More than 120 experienced home care physical therapists are on staff.
