UPMC/Jefferson Regional Home Health, LP is a limited partnership between the University of Pittsburgh Medical Center and Jefferson Regional Medical Center which began in October, 2000.

Together, our two organizations bring over 40 years of home health experience to the communities we serve. The level of quality and compassionate care provided by our staff has earned Joint Commission accreditation. As an industry leader, we provide individually focused and cost-effective home and community-based health services which achieve optimal patient satisfaction.

UPMC/Jefferson Regional Home Health covers ten counties in Pennsylvania and four in Ohio with an average daily census of 2,700 patients. Our organization makes over 386,000 visits per year and is Medicare certified in Pennsylvania and Ohio.

Specialized Nurses
Our specialized nurses teach you about your condition and how to follow a self-care plan. They help you with medication and symptom management. These nurses have advanced training in one or more specialties such as:

If you need rehabilitation to regain your best level of functioning, our physical, occupational, and speech therapists provide top-notch rehab services. And for stroke or joint replacement, we have a special Fast Track program to speed your recovery.

Nutrition Specialists
Our unique nutrition service lets you consult a nutrition specialist at your home or on the phone. Registered dieticians can assess and advise you about specific needs related to your health condition, such as diabetes, heart disease or obesity.

Wound Care Specialists
To heal wounds as quickly and painlessly as possible, our specialty-trained nurses work with the most current techniques and wound care products available. They will teach you how to comfortably follow a self-care plan to hasten your healing.

Behavioral Health Nurses
Our behavioral health nurses are specialists who can help you cope with a chronic illness and the changes it involves. They teach ways to manage stress through relaxation, provide grief counseling, and provide medication management. They are experts in dementia care.

Medical Social Workers
Our medical social workers know how to connect you and your family to the right services and resources in the community, whether you need a support group, an assisted living facility or financial aid.

Home Health Aides
During recovery, it may be diffcult or impossible to perform daily personal care. Our home health aides can help you with dressing, bathing, and other tasks. All have successfully completed a nursing assistant program.
